Hi @Gerard_La Foneria
This can occur when the database information about the files and the physical files themselves are not synchronised. This means that the database list of files (including quality and versions information) does not match the actual files on the hard disk.
The causes can be various: a data update, a change in the path of the media folder, a change of permissions in one of the folders, changes in the configuration of alternative qualities or versions, disk errors, etc.
If the data in the database about the media and the files do not match, this will also affect the publication.
In case you encounter such problems, it is recommended to do a massive ‘update cache’ from the corresponding media listing (audiovisual in your case).
I hope this helps
Best
